Alumni Questionaire
Complete this form to let Dallas Thunder know how you are doing and what you are doing.

Prayer Request & Praise Report Form
Use this form to request prayer or to make a praise report. Information will automatically be forwarded to Padre Chris.

Dallas Thunder Grievance Form
This form should be used to request a meeting with the Board of Directors in an attempt to resolve your unresolved grievance with a Coach. You should have, first, spoken to the Coach directly, in the appropriate environment. Second, had a meeting with the Coach and the Athletic Director.

Dallas Thunder Volunteer Form
Dallas Thunder counts on our volunteers to make our programs efficient and affordable. We ask that every family provide at least one parent to serve in a volunteer capacity. Use this form to let the organization know the areas you are interested in volunteering in.
